Name of Product: Realspace Soho Prestigio High-Back Leather Chair

Units: About 8,400

Manufacturer: True Innovations LLC, of Hong Kong, China

Importer: Office Depot Inc., of Boca Raton, Fla.

Hazard: The seat plate can break, posing fall and injury hazards to consumers.

Incidents/Injuries: Office Depot has received 78 reports of incidents involving seat plates breaking, including one report of a fall injury.

Description: This recall involves Prestigio model leather office chairs. The high-back leather chairs were sold in black and have a five-leg metallic finish base with casters. The SKU number 181-265 and the words “Realspace Soho” and “Prestigio High-Back Chair Black Leather” are printed on a label located on the underside of the seat.

Sold exclusively at: Office Depot retail stores nationwide and online at OfficeDepot.com from October 2008 through February 2011 for about $250.

Baby Seats Recalled Due to Fall Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Bumbo Baby Seats

Units: About 4 million in the U.S. Note: In October 2007, 1 million Bumbo seats were voluntarily recalled to provide additional warnings against use on raised surfaces.

Manufacturer: Bumbo International Trust, of South Africa

Hazard: Babies can maneuver out of or fall from the Bumbo seat, posing a risk of serious injuries.

Incidents/Injuries: CPSC and Bumbo International know of at least 50 incidents after the October 2007 voluntary recall in which babies fell from a Bumbo seat while it was being used on a raised surface. Nineteen of those incidents included reports of skull fractures. CPSC and Bumbo International are aware of an additional 34 post-recall reports of infants who fell out or maneuvered out of a Bumbo seat used on the floor or at an unknown elevation, resulting in injury. Two of these incidents involved reports of skull fractures, while others reported bumps, bruises and other minor injuries.

Description: The bottom of the Bumbo seat is round and flat with a diameter of about 15 inches. It is constructed of a single piece of molded foam and comes in various colors. The seat has leg holes and the seat back wraps completely around the child. On the front of the seat in raised lettering is the word “Bumbo” with the image of an elephant on top. The bottom of the seat has the following words: “Manufactured by Bumbo South Africa Material: Polyurethane World Patent No. PCT: ZA/1999/00030.” The back of the seat has several warnings and seats manufactured since 2008 have an additional label on the front of the seat warning against use on raised surfaces.

Sold by: Sears, Target, Toys R Us (including Babies R Us), USA Babies, Walmart, and various other toy and children’s stores nationwide, and various online sellers, from August 2003 through August 2012 for between $30 and $50.

Manufactured in: South Africa

Remedy: Consumers should immediately stop using the product until they order and install a free repair kit, which includes: a restraint belt with a warning label, installation instructions, safe use instructions and a new warning sticker. The belt should always be used when a child is placed in the seat. Even with the belt, the seat should never be used on any raised surface. Consumers should also immediately stop using Bumbo seat covers that interfere with the installation and use of the belt. Family Dollar Stores Recalls Decorative Light Sets

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with Family Dollar Services, Inc., of Matthews, N.C., today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: 200 Mini Lights

Units: About 280,000

Importer: Family Dollar Services, Inc., of Matthews, N.C.

Hazard: The light sets do not meet the UL standard for this product and pose a fire and shock risk.

Incidents/Injuries: There were three reports of overheating. No injuries or property damage were reported.

Description: The recalled decorative light sets have “200 Mini Lights” and “Multi Lights, Green Wire” printed on the red box. The product is identified by SKU # 2211428 and UPC # 049696720465 found on the back of the package. The product contains labels attached to a tag on the power cord with UL Listing # E346525 and Model # F0L200A4S.

Sold exclusively at: Family Dollar stores from September 2011 through December 2011 for $8.

Remedy: Consumer should immediately stop using the light sets and return the product to a Family Dollar store for a full refund.

 

Bosch Recalls SkilSaw Miter Saws Sold at Lowe’s

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the Robert Bosch Tool Corporation, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

 

Name of product: Miter Saw

 

Units: Approximately 22,149

 

Importer: Robert Bosch Tool Corporation of Mount Prospect, Ill.

 

Hazard: The lower guard can break and contact the blade during use, posing a laceration hazard to users.

 

Incidents/Injuries: The firm has received no reports of incidents or injuries.

 

Description: The recalled product is the SkilSaw® 10-inch compound miter saw, with model number 3316 and date codes 111, 112, 201, 202, 203 or 204. The model number and date code are on the lower right side of the name plate located on the motor housing. The SkilSaw logo appears at the top of the upper blade guard and on the dust collection bag.

 

Sold at: Lowe’s Home Centers nationwide and OC Tanner from January 2012 to April 2012.

Voluntary Limited Recall of Pedigree Brand Wet Dog Food

Mars Petcare US has announced a voluntary recall of a limited range of three varieties of PEDIGREE® weight management canned dog food products due to a potential choking risk.

Affected product may contain small pieces of blue plastic, which entered the food during the production process. The source of the plastic has been identified and the issue resolved. We encourage consumers who have purchased affected product to discard the food or return it to the retailer for a full refund or exchange. While a small number of consumers have reported finding the plastic pieces, we have not received any reports of injury or illness associated with the affected product. The lot codes indicated below should not be sold or consumed.

Affected product was distributed to retail customers throughout the United States. Mars Petcare US is working with all of our distributors and retail customers to ensure that the recalled products are no longer sold and are removed from inventory.

Only cans of PEDIGREE® weight management canned dog food varieties with the production codes shown below are included in this voluntary recall.  Each product will have a lot code printed on the end of the can that begins with 209, 210, 211 or 212 and a Best Before date that falls between 2/24/2014 and 3/23/2014.

Name of Product: Harbor Breeze Bath Fans with Heater and Light

Units: About 68,000

Manufacturer: Delta Electronics (Dongguan) Co. Ltd., of China

Hazard: The fan’s heater blades can fail to rotate properly, causing the fan to overheat and posing a fire hazard.

Incidents/Injuries: The firm has received 11 reports of the fan overheating with smoking or flames within the fan housing, including three reports of minor property damage.

Description: This recall involves plastic Harbor Breeze bathroom fans with a center light and a heater. The fans are white and measure about 11 x 17 inches. Model number 7109-01-L, Lowe’s item number 194492 and UPC code 820633985358 are printed on the fan’s packaging. A date code beginning with either 0, 11, 12 or 13 is printed on the fan’s housing, which indicates the fan was manufactured between August 2010 and March 2011.

IKEA Recalls Track Lighting System Due to Electric Shock Hazard

Hazard: The ground connection in the track is defective, posing an electric shock hazard.

Description: This recall involves white, straight track systems used to hold and power light fixtures. The recalled tracks are model numbers 00149242 and 10149246, supplier number 21338 and date stamp 1134 through 1213. The brand name IKEA, 365+ SÄNDA, supplier number and date stamp are printed on a white label attached to the side of the track. The recalled tracks are 28” and 45” long.

Strollers by Kolcraft Recalled Due to Fingertip Amputation and Laceration Hazards

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Contours Options three- and four-wheeled strollers

Units: About 36,000 in the United States and 270 in Canada

Manufacturer: Kolcraft Enterprises Inc., of Chicago Ill.

Hazard: A child or consumer’s finger can become caught in the opening formed when locking and unlocking the hinge mechanism which is used to adjust the handlebars on the strollers. This presents an amputation and laceration hazard to children and the adults handling the stroller.

Incidents/Injuries: Kolcraft and CPSC have received five reports of injuries involving the hinge mechanism, including reports of three children whose fingertips were amputated and two adults whose fingers were either smashed or lacerated.

Description: This recall involves Kolcraft Contours Options three- and four-wheeled strollers. Strollers included in the recall have model numbers starting with ZL002, ZL005, ZL008, ZL015 and ZL018. On the ZL002 model, the model number and date of manufacture is printed on a sticker above the left wheel. On the ZL005, ZL008, ZL015 and ZL018 models, the model number and date of manufacture is printed on a label sewn into the edge of back of the stroller seat pad. The strollers were manufactured from January 2006 through November 2009 and sold in various color schemes.

Sold at: Juvenile product specialty stores nationwide and online at Amazon.com, Target.com and ToysRUs.com from January 2006 and June 2012 for between $150 and $160.

Nautilus Recalls Bowflex Dumbbells

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with  Nautilus Inc. of Vancouver, Wash., today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Bowflex® SelectTech® 1090 Dumbbells

Units: About 17,000

Distributor: Nautilus Inc. of Vancouver, Wash.

Hazard: The weight selector dial on the units can fail, causing weight plates to fall when the dumbbell is lifted from its cradle, posing an injury hazard.

Incidents/Injuries: Nautilus Inc. has received 16 reports of the weight selector dial failing, including three reports of injuries to the user’s foot or leg.

Description: These black steel dumbbells are sold in pairs and can have a weight capacity of 10 to 90 pounds. The word ‘BOWFLEX’ is written in grey along the top of each handle. A selector dial on each end of the cradle lists weights in red, starting at 10 and increasing in increments of five. The red Bowflex logo is at the center of the dial. This recall involves units with a date-of-manufacture code in the serial number in the range of 1111 through 1136. The date code is the four-digit code following the letters “MAG” in the serial number. The serial number is on a white label on the bottom of the cradle. Lift the dumbbell from the cradle with all weights attached before turning over the base. Dumbbells with a black dot on the inside portion of the weight selector dial are not affected by this recall.

Sold at: Sporting goods stores, Nautilus.com and other online retailers from May 2011 through August 2011 for about $600.

Toys R Us Recalls Imaginarium Activity Center Due to Choking Hazard

Name of Product: Imaginarium 5-Sided Activity Center

Hazard: The small wooden knobs attaching the xylophone keys to the end can detach, causing a choking hazard to young children.

 

Incidents/Injuries: Toys “R” Us has received eight reports of the knobs detaching. No injuries have been reported to the firm.

 

Description: This recall involves the Imaginarium 5-Sided Activity Center. The multi-sided center has two triangle-shaped ends with a wooden multi-colored xylophone on one and a mirror on the other. The three sides are square-shaped and include moveable block letters, rotating gears and sliding shapes. The name “Imaginarium” appears in a blue, oval-shaped logo underneath the mirror on one end. The second ‘i’ in the logo is dotted with a shooting star. The recalled product has model number 46284; Toys “R” Us item number 295909; and has the barcode number 000799985462841. The model number is printed on the back of the box, directly above the bar code. These numerical markings do not appear on the actual product.

 

Sold exclusively at: Toys “R” Us stores nationwide and online via www.toysrus.com from August 2009 through September 2010 for about $25.
